What Is Rhinoplasty?
Surgery for reshaping the nose is called rhinoplasty. The goal of rhinoplasty may be to improve breathing or to change the appearance of the nose.
Bone makes up the upper portion of the nose, while cartilage makes up the lower portion. Rhinoplasty can alter bone or cartilage, or it can affect both. Consult your surgeon about whether you are a candidate for rhinoplasty and what it can accomplish for you.
As your surgeon plans your rhinoplasty, your other facial features, the skin on your nose, and what you want to change will be taken into account. When you are a surgical candidate for rhinoplasty in Iran, your surgeon will create a customized treatment plan for you.
A rhinoplasty may be covered in part or whole by insurance.
What Can Rhinoplasty Treat?
- The balance between the size and shape of the nose
- Whether the nose is wide at the bridge or how the nostrils are shaped and positioned
- Bridge of nose with visible humps or depressions
- An enlarged or bulbous nasal tip, drooping, upturned, or hooked nose
- Large, broad, or upturned nostrils
- Nasal asymmetry
It is important to remember that everyone’s face is asymmetrical to some degree if you desire a more symmetrical nose. Even though the goal is to create facial balance and correct proportion, the results may not be completely symmetric.
Getting Ready for a Rhinoplasty
Consult your surgeon first to determine whether you’re a good candidate for rhinoplasty. You will discuss why you would like the surgery and what you expect from it.
If you are on any medications or have a medical condition, your surgeon will ask about it. A surgeon may recommend that elective surgery not be performed if you have hemophilia, a disorder that causes excessive bleeding.
To determine what changes can be made to your nose, your surgeon will closely examine the inside and outside of the nose. Laboratory tests may also be ordered.
In addition, your surgeon will consider whether any other procedures need to be done concurrently. At the same time as rhinoplasty, some people also opt for chin augmentation, a procedure that enhances the appearance of the chin.
Photographs of your nose from various angles are also taken during this consultation. The shots will be used to assess the long-term effects of surgery and may be referred to during the procedure itself.
Ensure that you are aware of the surgery costs. It’s much less likely that an insurance company will cover cosmetic rhinoplasty.
Two weeks before your surgery and two weeks afterward, you should avoid using painkillers containing ibuprofen or aspirin. You may bleed more when taking these medications since they slow down the blood-clotting process. You should inform your surgeon of any medications and supplements you take so they can advise you if you should continue them.
Rhinoplasty recovery is more difficult for smokers, as cigarettes slow the healing process. In addition to causing your blood vessels to constrict, nicotine also causes your healing tissues to receive less oxygen and blood. It is therefore beneficial to stop smoking before and after surgery.
Procedure for Rhinoplasty
Depending on the procedure, you may have it in a hospital, doctor’s office, or outpatient surgery facility. You will be anesthetized either locally or generally. Your nose will be numbed with local anesthesia before a simple procedure, which will also numb your face. It is possible to receive medications through an IV line that make you dizzy, but you will still be awake.
You will become unconscious from a drug you inhale or receive through an IV during general anesthesia. A general anesthetic is usually administered.
Your surgeon will make cuts between or, if necessary, inside your nostrils once you are numb or unconscious. As soon as they separate your skin from your cartilage or bone, they can begin reshaping it. Your doctor may have to remove some cartilage from your ear or deep inside your nose if you need additional cartilage for your new nose. Implants and bone grafts might be necessary if more work is required. An extra bone is connected to the bone in your nose via a bone graft.
It usually takes one to two hours to complete the procedure. Complications can prolong the process.

Types of Noses in Terms of Rhinoplasty
Rhinoplasty can be performed on two different noses: the bony nose and the fleshy nose (meaty nose). Skin thickness distinguishes between bony noses and fleshy noses. Thick skin covers meaty noses, while thin skin covers bony noses.
With a bony nose, the skin is thin, and the details of its structures are more easily recognizable. Due to their stronger cartilage, the nasal tip is generally firmer and less likely to droop on these noses.
The thick skin of fleshy noses, however, conceals the bony structure and cartilage underneath. In addition, fleshy noses have thinner and weaker cartilage tissues.
However, it is possible that the above general rule does not apply; for example, a bony nose can have thick skin, and a fleshy nose can have thin skin with strong cartilages.

The Difference Between a Bony Nose and a Fleshy Nose
Rhinoplasty is performed differently for fleshy noses versus bony noses. Reshaping the tip of the nose poses a challenge in meaty nose surgery. A good result can be achieved when the sides and tip of the nose are reshaped in fleshy nose surgery.
A new technique is used to strengthen the nasal tip and prevent it from dropping due to the possibility of deformation of the lower part of fleshy noses after rhinoplasty due to their weak cartilages and plump skin.
A small change in the nose’s structure is likely to produce significant results in bony noses. It is not uncommon for bony noses to have little fat under the skin. Those with bony noses will experience minor swelling after surgery than those with fleshy noses.
If the nose is bony, the procedure must be carried out with extreme delicacy so that the outcome is symmetrical. To achieve the desired result from either a boney nose or meaty nose surgery, you will need to choose an experienced and skillful surgeon.

The Common Iranian Nose Job Styles
Many Iranian rhinoplasty surgeons and patients often describe different nose jobs as ‘fantasy’ (or ‘dolly’), ‘semi-fantasy, ‘or ‘natural.’
Natural nose job
The nose is straightened in the ‘natural’ rhinoplasty technique, resulting in a straighter nasal bridge than one with a curved bridge. Furthermore, a ‘natural’ nose job style does not lead to an extensive angle between the upper lip and nasal tip.
‘Natural’ style for bony noses
Dorsal humps on a nose make it a good candidate for the “natural” style.

‘Natural’ style for fleshy noses
In general, a ‘natural’ style of rhinoplasty is performed on fleshy noses.
Fantasy nose job (dolly nose) or barbie nose
Essentially, a dolly nose is an upturned, tiny, short nose (also called a ski-jump nose or a snub nose). An Iranian ‘dolly nose’ surgery involves making the nose small and upturned (so-called ‘retroussé nose’). It produces the most favorable results in bony noses and suits and is most appealing in faces with small features.
Semi-fantasy nose job
The ‘semi-fantasy nose job style lies somewhere between the ‘natural’ and ‘fantasy’ styles. The bridge is slightly curved, and the angle between the nose and the lips is narrower than that of a fantasy nose.
Women are more likely to have fantasy or semi-fantasy noses than men, who typically choose a natural style. The outcome of these two techniques is highly influenced by the skill and experience of the surgeon, in addition to the type of nose used.
Male rhinoplasty (men’s nose job)
Not only women can have a nose job or rhinoplasty. Rhinoplasty is becoming increasingly popular among men as they seek a better-looking nose. Male-rhinoplasty is not the same as female-rhinoplasty, even though this cosmetic surgery is becoming more and more popular for both genders.
The results of male and female nose jobs can differ due to differences in anatomy and preferences, even though plastic surgeons use the same surgical techniques during the procedure.
There are differences between men’s and women’s noses. The bridges of their noses tend to be straighter and more expansive, their skin is thicker, and their tips tend to be more bulbous and significant than those of women. Because men’s nose tips are less elevated, they tend to create an angle of 90 to 95 between their nose and upper lip.
While most women would like to get a small nose and slants upwards, most men would like a nose with a straighter angle and a more prominent nasal structure. The patient will feel as though he has a more masculine nose while feeling satisfied with his nose.
Rhinoplasty Recovery
Your doctor can place a plastic or metal splint on your nose following surgery. You can keep your new nose shape with the splint while it heals. A nasal pack or an ear splint may also be placed inside your nostrils to stabilize your septum.
In the recovery room, you will be monitored for several hours after surgery. Later in the day, you will be able to leave. Anesthesia will still affect you when you return home, so you’ll need someone to drive you. You may stay in the hospital for a day or two if you have a complicated procedure.
You will want to elevate your head above your chest while resting to reduce bleeding and swelling. Feeling congested may be caused by a swollen or clogged nose. Following surgery, splints and dressings are usually required to remain in place for a week. The stitches might be absorbable, so they won’t have to be removed. Your doctor will need to remove the stitches a week after the surgery if the stitches are not absorbable.
Medications used for surgery cause memory loss, impaired judgment, and slowed reaction times.
Your first night should be spent with a friend or relative if possible.
During the first few days following surgery, you might experience bleeding and drainage. Gauze drip pads, which are taped under the nose to absorb blood and mucus, can be used. You will be instructed when to change your drip pad by your doctor.
The pain medication your doctor prescribes might cause headaches, puffy skin, and a puffy face.
During the first few weeks after surgery, you may be advised to avoid the following:
- Strenuous physical activities, such as running
- Swimming
- Nose blowing
- Overeating
- Facial expressions that require lots of movement, such as laughing or smiling
- Pulling your clothes over your head
- Placing your eyeglasses on your nose
- Brushing your teeth vigorously
Don’t overexpose yourself to the sun. Your nose might become permanently discolored if you do.
Your return to work or school should take about a week.
For a few weeks after rhinoplasty, you may feel numb, swollen, or discolored around your eyelids. It’s possible that this can last for six months or even longer, and slight swelling could persist for many months. Discoloration and swelling can be decreased by applying cold compresses and ice packs.
After rhinoplasty, it is essential to follow up. Your doctor will guide you in how to proceed after your procedure.

Rhinoplasty Results
After rhinoplasty, the healing process can take a while, even though it is a relatively safe and easy procedure. You may experience numbness and swelling at the tip of your nose for several months if you have severe allergies. Some effects can linger for months after hospitalization, even if you fully recover within a few weeks. A whole year may pass before you can see the total outcome of your surgery.
