What is the process of eye examination in Iran?

Basic Eye Examination:

The examination begins by obtaining information about your state of health and family diseases. The doctor will most likely ask you to name the letters or numbers, shown in a table of letters and numbers arranged in rows and gradually decreasing in size. It allows your doctor to determine your central vision.

If you wear contact lenses, your eyesight will be tested at this point, the examination is done using a unique tool called a lensometer, and the doctor will check your glasses to confirm if the prescription was correct.

To test visual acuity, it is necessary to cover the other eye with a dense object (it is better not to cover the eye with fingers, the density of which may change during testing). When examining vision, patients try to read a table at a distance of 6 m. If this study is not possible, it is possible to check the patient’s vision using printed text or a specialized table located 36 cm from the patient’s eyes. Normal vision or vision with deviations is calculated according to the conventional signs of Snellen. The Snellen index of 20/40 (6/12) indicates that the smallest letters that a person can read with normal vision at a distance of 12 m must be transferred to 6 m to be recognized by the patient. Visual acuity is determined by the tiniest lines in which the patient can read half of the letters, even if they seem blurry or have to be guessed from the outlines. If the patient cannot read the most extensive line in the table from a distance of 6 m, the test must be repeated at 3 m. If the patient cannot read the lines on the table even from the closest distance, the following test must be performed: the doctor holds a different number of fingers in front of the patient to see if he can accurately count them. If not, it is necessary to check whether the patient can perceive the movement of the hand. If not, then the light is sent to the eye to determine if the patient perceives the light.

Near visual acuity is tested using a standard table or newspaper at a distance of 36 cm. Patients > 40 years of age wearing glasses or contact lenses should wear them to try near visual acuity.

Refractive error can be roughly estimated using a handheld ophthalmoscope, choosing the lens necessary to focus the retinal image; to do this, you must use your lenses to test your vision. This procedure is not a substitute for a comprehensive refraction assessment. Most often, refractive errors are assessed using a standard phoropter or an automated refractometer (a device that measures changes in the reflection of light projected onto a patient’s retina). The same instruments help assess the degree of astigmatism.


Examination of the eyelids and conjunctiva

The rims of the eyelids and the tissues surrounding the orbit are examined under focused light under magnification (For example, with a loop, slit lamp, or ophthalmoscope). If dacryocystitis or canaliculitis is suspected, the lacrimal sac is palpated, and an attempt is made to squeeze out its contents through the lacrimal ducts and points. Eyelid eversion can examine the conjunctiva of the eyelids, globe, and conjunctival fornix for foreign body, signs of inflammation (e.g., follicular hypertrophy, exudate, hyperemia, edema), or other abnormalities.


Cornea examination

Fuzzy edges or blurring of the light reflex (reflections of light from the cornea) are a sign of a violation of the integrity or compaction of the cornea’s surface, for example, with corneal erosion or keratitis. Erosions and ulcers of the cornea can be detected using fluorescein staining. If the patient is in pain or if there is a need to touch the cornea or conjunctiva (for example, to remove a foreign body or measure IOP), local anesthesia with a solution of procaine 0.5% or tetracaine 0.5 may be used to facilitate the examination procedure for the patient. A sterile fluorescein test strip is moistened with a drop of sterile saline or local anesthetic and applied to the inside of the lower eyelid while the patient is looking up. The patient then blinks several times to spread the color over the tear film as the examination is performed under magnification in cobalt blue light. Areas lacking epithelium on the cornea or conjunctiva (in case of erosion or ulceration) are fluorescent red.
